Pathways Center for Behavioral & Developmental Growth
provides each consumer with an informed consent document
and
authorization form whereby each individual receiving
treatment and services from Pathways Center agrees to
participate in the determined individual treatment plan,
understands the risks and/or benefits, and understands
how the confidentiality of records and personal health
information (PHI) will be protected.
Pathways Center for Behavioral & Developmental Growth takes
the privacy of its consumers seriously and as such will
only
disclose personal health information (PHI) to others
within the Pathways Center organization or to other
agencies complying with the Privacy regulations if it is
for the consumer’s treatment or payment of services
rendered.
